Coach-Mentors supports individuals through customized, in-home and community-based services that allow them to remain close to family and familiar supports. Levels of care are adjusted to match individual needs, ranging from periodic weekly support to full-time, live-in assistance. When families require flexible or nontraditional services beyond what typical programs offer, Coach-Mentors creates responsive support plans tailored to higher levels of complexity or intensity. This approach provides a viable alternative to residential placement by delivering coordinated, individualized services that promote growth, stability, and long-term success within the client’s home and community environment.
