The program is available nationwide and begins with an intensive four-week diagnostic and functional assessment period, followed by customized monthly services.
The first month of CarePathways includes approximately 15 distinct assessments. These evaluations range from psychological and trauma screenings to executive functioning, neurodivergent, nutritional, and legal evaluations, and are combined with 15 hours of clinical case management. This thorough onboarding process ensures that care is based not on guesswork, but on real data tailored to each individual's biopsychosocial profile. Following this foundation, clients transition into an ongoing support model that provides up to 30 hours of integrated care per month, including therapeutic, medical, dietary, and family-centered interventions. All services are coordinated through a clinically supervised, interdisciplinary team that ensures continuity, responsiveness, and transformational outcomes.
CarePathways is ideal for individuals stepping down from residential care, those with complex co-occurring conditions, or clients and families in need of a structured, high-touch alternative to traditional outpatient care.
